The Bureau of Quarantine (BOQ) of the Department of Health (DOH), as a first-class line bureau, shall have a national scope of function and international commitment about the World Health Organization’s International Health Regulations (IHR) (WHO).

Additionally, the bureau is empowered to issue rules and regulations as necessary to prevent the introduction, transmission, or spread of “public health emergencies of international concern” into the Philippines from foreign countries.

The bureau’s mission is to prevent the introduction and spread of infectious diseases and public health emergencies of global concern (PHEIC).

One of the intervention strategies used by the bureau to carry out its mandate is vaccination of international travelers and the issuance of corresponding health certificates.

Following the R.A. 11032, also known as the “Ease of Doing Business and Efficient Government Service Act of 2018,” the bureau implemented the Bureau of Quarantine Online Booking System (OBS) to provide quick and efficient service.

The BOQ has updated its BOQ Online Booking System (BOQ OBS) and created a new format for the International Certificate of Vaccination.

It includes a unique Q.R… code that allows other domestic and international stakeholders to verify the certificate’s authenticity.

It provides vaccinations, international certification, and physical examinations.

Clients may schedule an appointment under International Certification to obtain a duplicate or reissued ICV, International Certificate for Polio, or International Certificate for COVID-19.

Each certification is ₱300. It costs ₱240 for senior citizens.

The WHO created the International Certificate of Vaccination or Prophylaxis (ICVP), also known as the Carte Jaune or Yellow Card.

It is a travel document, akin to a medical passport, that is internationally recognized.

Additionally, it may be required for entry into certain countries where travelers face increased health risks.

The BOQ introduced the new International Certificate of Vaccination card on July 19, 2021. (also known as vaccine passport.)

The bureau now issues ICV for COVID-19 in addition to ICV for Polio.

The vaccine passport contains accurate information about an individual’s vaccination status, making it a reliable proof of vaccination against COVID-19.

Individuals who are fully vaccinated and intend to travel internationally may request an International Certificate of Vaccination (ICV) from the bureau, provided they adhere to specific guidelines.

Guidelines and Requirements

  1. International travelers must be immunized against COVID-19 in their home Local Government Unit (LGU) or at designated vaccination centers (bakuna center, hospitals, and other institutions).
  2. The local government units (LGUs) or special vaccination centers are responsible for submitting the master, and line lists of international travelers vaccinated to the DICT-managed Vaccine Information Management System.
  3. The international traveler must submit a request for a digital version of their certificate (DICT Department Memorandum).
  4. Suppose the international traveler does not have a VIMS-IR record. They must then proceed to their respective LGU or special vaccination centers, such as bakuna centers, hospitals, and other Department institutions, to upload their records to the master list.
  5. Suppose the international traveler has already departed and their records have not been updated. In that case, they may seek certification from their local government unit or special vaccination centers, such as bakuna centers, hospitals, and other institutions that verify the validity of their vaccination card. The certification mentioned above must include the following information:
  • Letterhead of the LGU or special vaccination centers i.e.
    bakuna center, hospitals, and other institutions
  • Name of the vaccine
  • Date of administration
  • Vaccine administered
  • Manufacturer of the vaccine administered
  • Lot/Batch number
  • Name, designation, and signature of certifying official

Currently, the BOQ only issues ICV for COVID-19 vaccines that have been recognized by the World Health Organization, which include the following:

  • AstraZeneca
  • Pfizer
  • Johnsson & Johnsson’s Janssen
  • Moderna
  • Sinovac
  • Sinopharm

Before booking an appointment, the client must upload a copy of their vaccination card issued by their respective local government unit or particular vaccination center, e.g., bakuna center, hospitals, or other institutions.

Additionally, they must bring the digital vaccination certificate with them on the appointment (if available).

How to book an appointment?

  1. Select your preferred location on the Online Booking System official website.
  2. After reading the Terms and Privacy Statement, click “I Agree to the Terms and Privacy Statement of this Website” to continue.
  3. Click “Book an appointment.”
  4. Put your email address, and click “Proceed.”
  5. Select “Certification” from the “Select Service” menu.
  6. Click “International Certification of Vaccine for COVID-19, and click “Next.”
  7. Select an available date and time for an appointment.
  8. Fill out all necessary information.
  9. Upload the necessary documents, including a valid I.D…, a senior citizen I.D…, and proof of immunization.
  10. Fill out all necessary information for payment.

How to verify appointment status?

  1. Select your preferred location on the Online Booking System official website.
  2. Click “View appointment status.”
  3. Then enter your reference number and date of birth in the appropriate fields, and click Submit to await the outcome of your appointment.
